County & District Evaluation Forms: Evaluation forms below are provided as suggested formats for use at county and district level Public Presentation events. 4-H Educators may choose to use an alternate form with or without numerical scoring. All forms used at county and district level events should continue to consider age and experience level and include similar terminology as used in the forms below.
